After the tulip review is approved, send a letter to the customer to confirm and collect the acceptance fee

The customer provides the email address of the source hospital, and Tulip sends a letter to the other hospital to coordinate necessary documents

Scanned copy of the assisted reproductive license from the other hospital (non English, additional English translation required);
Scanned copy of the confirmation signature document (transfer confirmation letter/consent form) for the customer to complete the transfer out procedure at the source hospital; Medical reports related to the transportation of materials:
Frozen embryo: test tube certificate in IVF cycle, including but not limited to: case summary, description of embryo sperm and egg source, transfer record (if any), discharge summary, embryo report, preimplantation genetic test report (PGT - A/MSR, if any) ④ report of infectious diseases (HIV/syphilis/hepatitis B/hepatitis C) between sperm and egg

After receiving a response that the tulip review has been approved, send a confirmation letter of receipt to the source hospital and begin the biological material transfer procedure

The transfer procedure begins, and the source institution or customer needs to send the "Biological Material Transfer Contract (Agreement)" to Tulip for filing
